Adult Day Program Coordinator

The Coordinator is tasked with helping develop and grow the EVOLVE Program. The program is designed to help young adults with neurodiverse abilities achieve their hopes and dreams of greater independence, vocational opportunities, and living a more full life.

This is a working role and will collaborate with a small team to coordinate and provide services to Wide Horizons’ program Participants including but not limited to program site management, classroom instruction, vocational preparation, employment counseling, mentorship, parent/caregiver engagement, and community service activities.

Minimum Qualifications:
• Three (3) years of experience in a human services delivery system
• and
• At least one (1) year in a comparable program
or
• A bachelor’s degree in a human services related field
• Demonstrated ability to provide staff training, supervision and planning.
• Fingerprint clearance from the Department of Justice or the Department of Social Services
• Signed Criminal Record Statement
• TB & health screening (not older than 12 months)
• Maintain current CPR certification
• Maintain current First Aid certification
• Obtain and maintain Behavioral Training certification (if applicable)
• Obtain annual appropriate continuing education training as needed to provide services

Statement of Duties:

• Planning, managing, coordinating and evaluating assigned staff efforts to achieve the program’s objectives and Participant outcomes
• Coordinating with the regional center for the implementation of Participant IPP objectives for which the vendor is responsible
• Assessing the program’s effectiveness in achieving Participant IP objectives
• Identifying barriers to Participant success in obtaining the IPP/ROADMAP objectives and how those will be overcome
• Organizing and monitoring the intake and continuing assessment process
